SUAA can connect you with others who have been where you are and help ensure you receive all of the benefits you are entitled to.

Here are a few basic steps that normally apply in these circumstances: when a SURS Member passes away, promptly notify SURS at 217-378-8800, or go to www.SURS.org and click on Life Events.

These documents should be on file with SURS:

  • updated beneficiary form with current addresses
  • annuitants birth certificate
  • survivor’s birth certificate
  • all marriage certificates and divorce papers
  • power of attorney, adoption, and guardianship papers

SURS will send you the necessary forms and information to ensure that survivors receive correct pension and healthcare benefits.
